PEN-Plus Monitoring and Evaluation Toolkit
The PEN-Plus Monitoring and Evaluation Toolkit outlines essential monitoring and evaluation components of PEN-Plus programs. The toolkit also provides links to key resources and tools—such as a monitoring and evaluation roadmap, a data dictionary, and PEN-Plus patient forms—and offers guidance on using those resources and tools.
The toolkit’s principal intended audiences are implementing facility staff and partners, prospective facility implementers, government monitoring and evaluation teams, technical partners, data system vendors, and others looking to support the worldwide expansion of care for people living with severe, chronic noncommunicable diseases in resource-limited settings.

The NCDI Poverty Network maintains the PEN-Plus Monitoring and Evaluation Toolkit as a living document that changes and adapts as PEN-Plus systems strengthen and higher levels of care and standards are achieved.
